Veterans Service Records and Medals



I would like to...

  • Obtain service records (two options for submitting request):
    • View online
      • Follow prompts, selecting the appropriate information, and advance by clicking on the “Continue” button on the right.
    • Fax/Mail
      • Download the Standard Form 180 (SF-180) to Request Military Records.
      • You may either complete the form online, or print and complete.
      • Review the tables on page 3 of SF 180 to identify the correct location of the record you need (based on branch of service, dates of separation, and type of record).
      • Send the completed form to the address identified on the table.
    • Emergency Requests
      • If your request is urgent (for example, upcoming surgery, a funeral, etc.) indicate the nature of the emergency and your deadline in the "Comments" section of eVetRecs or in the "Purpose" section of the Standard Form (SF) 180, Request Pertaining to Military Records.
        • Call the National Personnel Records Center (NPRC) customer service staff at (314) 801-0800 if you have questions or require same-day service. ***Due to the large number of calls we receive at this number, hold times are often long. Call your nearest district office, and we may be able to help.
  • Obtain service medals:
    • View online
      • Are you a member of the Coast Guard?
        • Submit requests for Coast Guard awards by mail to:

          Arlington, VA 20598-7200
      • Follow prompts, selecting the appropriate information, and advance by clicking on the “Continue” button on the right.
      • On STEP 2 there are 5 pieces of information required. The fourth (4) piece is “the most appropriate category for your request.
      • Select Military Awards/Decorations, and proceed.
  • Obtain a Cold War Recognition Certificate:
